Physical Exam Requirements

   Your healthcare provider must review the letter from our school nurse prior to the physical exam

• Health History Update Form - This form is only required when the physical exam is completed more than 90 days prior to the first day of official practice. A new update form is required for each season.  If your student plays multiple sports in one season, list the sports played on one form
